South Carolina Medicaid: Healthy Connections

As of January 1, 2026, “Healthy Connections Prime,” a previous plan for those with both Medicare and Medicaid, has ended, and members are being transitioned to a new Dual Special Needs Plan (D-SNP).

South Carolina Healthy Connections is the name for the state’s Medicaid program that helps pay for some or all medical bills for eligible low-income individuals and families.

It provides comprehensive health coverage to eligible low-income residents in South Carolina, including children, pregnant women, seniors, and people with disabilities.

South Carolina is a non-expansion state, meaning eligibility for non-disabled, adults without dependent children remains very restricted.
