Iowa Medicaid: Iowa Health and Wellness Plan (IHAWP)

Iowa Health and Wellness Plan (IHAWP) is a Medicaid program for low-income residents in Iowa, providing comprehensive health coverage, including primary care, hospital services, and prescription drugs.

There are no out-of-pocket costs for the first year and depending on family income, a small monthly contribution might be required on the subsequent years.

To continue receiving free coverage after the first year, you must complete one of two wellness exams — either a health or dental exam — and complete a health-risk assessment, collectively known as Healthy Behaviors.

What are the new Medicaid rules in Iowa?

Iowa’s proposed Medicaid work requirements would mandate that most adults ages 19—64 on the Iowa Health and Wellness Plan work or volunteer at least 80 hours each month to keep their coverage.1

Under the federal framework, full compliance checks for most “able-bodied” adult recipients are scheduled to begin by January 1, 2027.

Starting in January 2026, non-exempt members in certain counties may need to meet monthly “community engagement” requirements to maintain coverage.2
